High Needs High Cost members: Decreased emergency room visits by creating advanced member outreach and provider incentive program for commercial population.

Meals as Medicine: Collaborated with nonprofit community provider and commercial hospital system in implementing medically tailored meals program to prevent readmissions of patients with chronic conditions and expanding patient participation with extension into ambulatory clinics.

Community Centered Care: Crafted community collaborations and alliances to advance preventive services through public / private partnerships, winning inaugural Root Cause Coalition award (Colorado Prevention Alliance, 2018).

Organizational Initiatives

Pioneered first Diabetes Prevention Program in Anthem prior to USPSTF recommendation.

Completed initial in-home hospitalization program implementation in Anthem markets for commercial and Medicare Advantage markets to reduce admissions.

Increased member engagement 40% by executing on programs that addressed social determinants of health with nonclinical navigators and value-based incentive provider payments in a commercial population.
